After this period, unclaimed certificates will not be retained, and reprint fees will apply should a certificate be requested. • Certificate Reprint Request: Candidates that have lost their original NDAEB paper certificate or wish to request a reissuance of the NDAEB certificate because of a name change, can apply for a new paper and digital NDAEB certificate using the online NDAEB Certificate Reprint Application. o Please be advised that the certificate reprint fee and credit card processing fees are non-refundable. Candidates are responsible for reviewing their NDAEB profile information. Names must match as on government issued identification (nicknames are not acceptable). Incorrect information provided to the NDAEB may result in a reprint of the NDAEB Certificate and the candidate will be responsible for this additional fee. Regulatory Requirements Provincial Licensure /Certification/Registration Provincial regulations apply to the dental assisting profession in most Canadian jurisdictions. The NDAEB Theory Exam and Clinical Practice Evaluation may only be part of the licensure/registration/certification requirements in a province. You are advised to contact the CDARA in your province for regulatory information (find more information on the NDAEB website under Registration & Licensure). Returning to Practice A CDARA may require a dental assistant to retake the CPE upon re-entry to practice after a period of time away from the profession. Contact the CDARA for information.
