© 1998 – 2025 National Dental Assisting Examining Board 4 Requalification Policy: Candidates who have been unsuccessful on three (3) attempts may pursue one of two requalification pathways: 1. New Application Status: Candidates may requalify by successfully re-completing a recognized dental assisting program that includes all required theoretical and clinical components and is deemed exameligible by the NDAEB. 2. Fourth Attempt Request Based on Remediation: Candidates may request approval for a fourth examination attempt without re-entering a full educational program, subject to meeting specific conditions outlined below. i. Submit a written request form and fee to the NDAEB Registrar (or delegate) within 125 business days of the date of their third unsuccessful examination attempt. ii. Clearly set out the reason(s) why they believe a fourth attempt should be approved. iii. Provide a detailed remediation plan, outlining the steps that will be taken to address identified areas of weakness and improve examination readiness. iv. Provide any supporting documentation relevant to their remediation activities (e.g., courses, tutoring, practice tests). v. If request is accepted, candidates must submit to the NDAEB documentation/evidence that they have successfully completed the remediation plan within the stipulated time frame. vi. Upon review of the documentation, the NDAEB may grant a fourth and final attempt on the Theory Examination. Candidates whose requests are approved must successfully and completeyl carry out the remediation plan prior to applying for the fourth and final attempt on the Examination. Candidates whose requests are denied must successfully completea recognized dental assisting program to requalify for certification eligibility.
