NDAEB GNRP Application Checklist Revised March 31, 2026 • Certified/notarized true copies of the original academic transcripts and diploma. [Ensure you have these documents ready prior to starting the application process] o Copies of academic transcripts and diploma and other supporting documents issued inside and outside of Canada must be certified/notarized as true copies of the original by a notary public, lawyer or commissioner of oaths in Canada. If the original documents are not in English or French, translated copies must also be uploaded and must be certified/notarized as true copies in Canada by a notary public, lawyer or commissioner of oaths. Documents notarized in another country are not acceptable except those that were notarized by Canadian staff at a Canadian embassy or consulate abroad. o Translated documents will ONLY be accepted if certified/notarized in Canada by a notary public, lawyer or commissioner of oaths. *Digital or online notarizations are not accepted. * Please do not upload unnotarized documents to your application. You will be contacted and your application will remain pending. Program Survey Requirements Form must be completed by all applicants. The NDEAB will contact your school accordingly to request a program survey clearly indicating the mandatory core skills required for NDEAB eligibility were included in the program. Applicants must be aware that their NDAEB application status will remain as “Pending” until the NDAEB receives a completed Program Survey from their educational institute. *This survey is a requirement of eligibility. o Important Notice: Applicants must ensure the name, title and direct contact email of their program head is included in the Program Survey Requirements Form. • Proof of name change is required if the name appearing on the academic records or other documents submitted to the NDAEB does not match the name used in your application (e.g., abbreviated, initials only, or different spelling), a copy of a marriage or divorce certificate, affidavit or proof of legal name change is required for the NDAEB to verify and reconcile the name on the application with the name appearing on the academic records and other documents submitted. Documents issued inside and outside of Canada must be certified/notarized by a notary public, lawyer or commissioner of oaths in Canada. If the original documents are not in English or French, translated copies must also be uploaded and must be certified/notarized as true copies in Canada by a notary public, lawyer or commissioner of oaths. *Digital or online notarizations are not accepted. • Deadline for Assessments: Documents required for credential assessment must be received by the NDAEB office at least three (3) weeks prior to the CPE application registration day. Documents received after the GNRP deadline will not be guaranteed assessment for NDAEB eligibility for the applied CPE sitting. • Timeline for Assessment Approvals: Please allow 4-6 weeks for review. If you are missing documents, you will be contacted. *Any missing or incorrect documents will delay your assessment. *Important Notice: Applicants are advised to retain their original notarized documents, as they may be requested to mail them to the NDAEB office for verification and authentication purposes. Your application will remain as “Pending” status in the event the NDAEB is unable to verify the validity of the notarized uploaded copies. If needed, you will be contacted by NDAEB with further directives. *Please have a credit card ready for payment upon application as the form is time sensitive.
