Clinical Practice Evaluation Application Form • 202 How to Apply for the CPE Submit a CPE application form online, with a passport like photo and full payment (made payable by credit card, in Canadian funds) to be received by the NDAEB no later than 4:30 p.m. Eastern Time on the application deadline date. Incomplete and/or late applications will not be processed. Candidates are encouraged to submit applications as early as possible to ensure timely registration. Spots are limited so register early. Registration for the CPE is on a first come first served basis. Registrations received by the NDAEB will be time stamped. Site maximums are in effect. Once the maximum number of registrants is received, all others may be placed on a waiting list or returned. Incomplete and/or late applications will not be processed and will be returned to the applicant. Non-refundable fees will apply. NDAEB does not accept applications by fax machine or email. Note: • Applicants must have successfully completed the NDAEB written examination or hold the NDAEB Certificate prior to being eligible to apply for the CPE. • Candidates must apply for all skills required to complete the CPE. Candidates may not apply for partial CPE unless applying for reevaluation of individual skills or if proof of partial education in lieu of CPE is submitted with the application. Candidates enrolled in education upgrading at the time of registration, must show proof that their course/program will be completed prior to the date of their clinical evaluation. If education is not completed prior to a candidate's registered CPE, they will not be eligible and will automatically be withdrawn. Note: Due to the high demand for the CPE, candidates are not permitted to register for CPE in order to hold a place while awaiting confirmation to attend formal education upgrading (in lieu of the skills registered for CPE). Withdrawing from the CPE as a result of completing education upgrading in lieu of registered CPE skills, will result in withdrawal fees of at least 50 % of CPE fees paid. Testing Accommodations Requests for testing accommodations (form available on our website in the Document Library) must be received with applications by the NDAEB no later than 4:30 p.m. Eastern Time on the application deadline date. Written verification of the circumstances requiring accommodation, eg. special needs, medical conditions or alternate date for religious reasons, must be supported by reports, or other documentary evidence, signed by a medical practitioner, clinical psychologist or religious leader. The NDAEB reserves the right to determine what conditions/situations constitute a valid request for testing accommodations. Candidates should note that dependent on the availability of clinics in various locations in Canada, requests for special sitting dates due to religious reasons may not be possible at every sitting date. Note: • Prayer Times will be observed. Candidates must submit the form “Request for Testing Accommodations” stating the time(s) required for prayer and the duration to facilitate scheduling by NDAEB staff. • Candidates previously approved for testing accommodations with the written examination and/or CPE session, must submit a new ‘Request for Testing Accommodations Form’ with each application should they wish to have accommodations arranged. Evaluation Date and Confirmation The NDAEB schedules CPE sessions quarterly. Exact dates may vary at sites across Canada. Visit the NDAEB website www.ndaeb.ca. for details regarding dates, times and locations. Candidates will receive an email 2-3 weeks before their scheduled CPE session directing them to the candidate portal where they can access their Admission Letter. The Admission Letter will confirm their scheduled evaluation date, time and place to report.
