Clinical Practice Evaluation Application Form • 202 Note: • Due to scheduling purposes, candidates will not be permitted to change their selected evaluation site after the application deadline date. • Candidates are not permitted to choose their evaluation day (Saturday or Sunday). Requests for alternate days must be made by submitting a Requests for Accommodations Form with your CPE application. All requests must be received prior to the application deadline and must be approved by the NDAEB office. Candidates must be aware that due to high demand and clinic availability, evaluations are usually scheduled on weekends, including Saturday and Sunday. Session dates may also include the preceding Friday or Monday following the scheduled weekend. Please note: candidates are scheduled for one day only, and do not get to select which day their CPE session is scheduled. Candidates must also be aware that due to reasons beyond the NDAEB’s control, evaluation dates at specific sites and/or dates may change after the application deadline. In such cases, the NDAEB will advise candidates as soon as possible. While scheduling changes rarely occur, candidates should plan accordingly. Candidates are responsible to fully read all email correspondence and documentation posted to the Candidate Portal. Failure to do so may result in a missed session and all fees will be assessed.
